Dr. R.J. personally reviews and plans every referred case in coordination with the referring dentist. After the surgical phase, the patient returns to your office for restoration—or, at your request, Dr. R.J. can manage the case through final restoration.
Implant Planning and Surgery, Coordinated in One Office
Restore focuses on implant dentistry, with CBCT imaging, surgical planning, and implant treatment coordinated in one office. Dr. R.J. Guideng personally reviews each referred case and builds the treatment plan himself. He then reviews the plan with the referring dentist, performs the surgery, and sends the patient back to the referring dentist for restoration. If the referring dentist prefers, Dr. R.J. can instead manage the case from the scan through the final restoration.

Fill in the form and we take it from there. We contact the patient directly to arrange their consultation, and you get a written report once treatment is planned.
Please use the referral form for initial contact. Forms and supporting records can be sent to the office by email. If secure fax is needed, please contact our office for current fax instructions.
What to Include
The referral form takes a couple of minutes and there is no packet to assemble. We gather what we need at the consultation, so a case never sits waiting on paperwork from your office.
The patient's name and contact details.
A brief note on what you are referring for.
Recent radiographs if you have them, though they are not required.
Nothing further. We take our own CBCT scan as part of the consultation.

Restore is an implant-focused studio. The cases we typically do not take are listed alongside, helping you identify the right referrals before the patient is scheduled.

If a case falls outside our scope, we say so in writing rather than treating around the edges of it.
Just the patient's contact details and a short note on what you are referring for. The form covers it. Anything else we need, we gather at the consultation.
Not required. We take our own CBCT scan as part of the consultation, so there is no need to arrange imaging first. If you already have recent radiographs and want to send them, you are welcome to.
Our team reaches out to the patient directly once the referral comes through, so you are not left chasing an appointment or coordinating between two offices.
Yes. After the agreed treatment phase, the patient returns to the referring practice for restoration and ongoing general and hygiene care. If the referring dentist asks Dr. R.J. to complete the restoration, the patient returns to the referring practice for ongoing care afterward.
The referring dentist normally completes the restoration after Dr. R.J. performs the surgical phase. If the referring dentist specifically prefers, Dr. R.J. can manage the case from the scan through the final restoration.
We tell you. If the scan shows the case is not one we should take on, or a different approach makes more sense, you get that in the written report rather than finding out second-hand from the patient.
Yes. Implant complications are one of the things we see most, including work placed at other practices. Referring a case that is not going to plan is treated as a clinical problem to solve, not a judgment on the referrer.
The patient is quoted directly by Restore before treatment is scheduled. A straightforward zirconia full arch is $17,999 for either the upper or lower arch. More difficult or complex cases are priced appropriately after the consultation and treatment planning.
